Role Overview:

In this role, you will provide a range of primary care services, from diagnosing and managing acute illnesses to supporting patients with long-term conditions. You will be part of a multidisciplinary team and use EMIS Web to ensure efficient and accurate patient record management.

Key Responsibilities:

• Patient Consultations: Conduct face-to-face, telephone, and video consultations to diagnose and manage a wide range of health conditions.

• Chronic Disease Management: Develop personalised care plans for patients with conditions such as diabetes, asthma, and hypertension.

• Preventative Care: Promote health education, screenings, and preventative care to support patient wellbeing.

• Prescribing: Prescribe medications safely and effectively in line with clinical guidelines.

• Record Keeping: Maintain accurate and detailed patient records using the EMIS Web system.

• Team Collaboration: Work closely with nurses, healthcare assistants, and administrative staff to ensure seamless patient care.

• Clinical Development: Participate in clinical audits, quality improvement initiatives, and training sessions to enhance service delivery.
