Are you a dedicated Clinical Pharmacist with experience using EMIS Web? We are seeking a passionate and detail-oriented pharmacist to join a forward-thinking GP practice in Dorset. About the Role:
As a Clinical Pharmacist, you will play a crucial role in optimising medication management and improving patient outcomes. You will support the multidisciplinary team in delivering high-quality care by conducting medication reviews, providing expert advice on prescribing, and helping patients manage their treatment plans effectively.
Key Responsibilities:
Medication Reviews: Conduct structured medication reviews to ensure safe and effective use of medicines.
Prescribing Support: Provide guidance on appropriate prescribing practices and medication optimisation.
Patient Consultations: Support patients in managing long-term conditions and provide advice on medication adherence.
Repeat Prescriptions: Manage repeat prescribing systems and resolve queries related to medication.
Clinical Audits: Participate in audits and quality improvement initiatives related to medicines management.
Record Keeping: Accurately document all consultations and interventions using the EMIS Web system.
Collaborative Care: Work closely with GPs, nurses, and other healthcare professionals to ensure integrated patient care.

Requirements:
GPhC Registered Pharmacist with a relevant postgraduate qualification in clinical pharmacy (or working towards one).
Experience working in primary care or general practice settings.
Proficiency in using EMIS Web for patient records and medication management.
Strong clinical knowledge and understanding of medicines optimisation.
Excellent communication and interpersonal skills.
Ability to work independently and collaboratively within a multidisciplinary team.
