About the Role:

As a Clinical Pharmacist, you will play a crucial role in optimising medication management and improving patient outcomes. You will support the multidisciplinary team in delivering high-quality care by conducting medication reviews, providing expert advice on prescribing, and helping patients manage their treatment plans effectively.

Key Responsibilities:

• Medication Reviews: Conduct structured medication reviews to ensure safe and effective use of medicines.

• Prescribing Support: Provide guidance on appropriate prescribing practices and medication optimisation.

• Patient Consultations: Support patients in managing long-term conditions and provide advice on medication adherence.

• Repeat Prescriptions: Manage repeat prescribing systems and resolve queries related to medication.

• Clinical Audits: Participate in audits and quality improvement initiatives related to medicines management.

• Record Keeping: Accurately document all consultations and interventions using the EMIS Web system.

• Collaborative Care: Work closely with GPs, nurses, and other healthcare professionals to ensure integrated patient care.

Requirements:

• GPhC Registered Pharmacist with a relevant postgraduate qualification in clinical pharmacy (or working towards one).

• Experience working in primary care or general practice settings.

• Proficiency in using EMIS Web for patient records and medication management.

• Strong clinical knowledge and understanding of medicines optimisation.

• Excellent communication and interpersonal skills.

• Ability to work independently and collaboratively within a multidisciplinary team.
