About the Role:
In this role, you will be a vital part of the clinical team, providing expert care to patients and supporting the overall operation of the practice. Your proficiency with EMIS Web will ensure you can seamlessly manage patient records and care plans. You’ll work closely with GPs, practice nurses, and other healthcare professionals to provide holistic care, helping patients manage their health effectively.

Key Responsibilities:

• Clinical Assessments: Perform comprehensive assessments and diagnose a wide range of acute and chronic conditions.

• Treatment Planning: Develop and implement evidence-based care plans tailored to individual patient needs.

• Prescribing: Independently prescribe medications and treatments (V300 qualification required).

• Urgent Care: Manage same-day appointments, minor injuries, and acute illness presentations.

• Health Promotion: Educate patients on preventative healthcare, lifestyle changes, and chronic disease management.

• Record Keeping: Maintain accurate, up-to-date patient records using the EMIS Web system.

• Team Collaboration: Work alongside a multidisciplinary team to ensure comprehensive patient care and support practice efficiency.

• Mentorship: Support and mentor junior staff and nursing colleagues, fostering a culture of continuous learning.

Requirements:

• NMC Registered Nurse with an Advanced Nurse Practitioner qualification.

• Independent Prescribing Qualification (V300).
