About the Role:
As an Advanced Clinical Practitioner, you will play a vital role in delivering high-quality, patient-centred care. You will be responsible for assessing, diagnosing, and treating patients across a range of acute and chronic conditions, working closely with GPs, practice nurses, and other healthcare professionals to ensure a holistic approach to care.
Key Responsibilities:
Clinical Assessment & Diagnosis: Conduct thorough patient assessments and diagnose a range of conditions.
Treatment & Prescribing: Develop and implement evidence-based treatment plans, prescribing medications independently (V300 required).
Urgent & Primary Care: Manage minor illnesses, injuries, and long-term conditions with confidence and autonomy.
Health Promotion: Educate and advise patients on preventative care, health promotion, and lifestyle management.
Documentation: Accurately record patient interactions and treatment plans using the EMIS Web system.
Collaboration: Work alongside GPs, nurses, and other healthcare staff to deliver cohesive patient care.
Leadership & Mentoring: Support the development of junior staff and nursing colleagues, fostering a culture of continuous learning.

Requirements:
NMC Registered Nurse or HCPC Registered Professional with an Advanced Clinical Practitioner qualification.
Independent Prescribing Qualification (V300).
Proficiency in using EMIS Web for patient record management.
Proven experience in primary care, urgent care, or community healthcare settings.
Strong clinical decision-making, communication, and interpersonal skills.
Ability to work autonomously and as part of a multidisciplinary team.
