Job description
Are you a skilled Advanced Clinical Practitioner (ACP) with a passion for patient care and experience using Adastra? We are excited to offer an excellent opportunity to join a forward-thinking and supportive GP practice in Portsmouth, a historic coastal city known for its maritime heritage, vibrant community, and excellent quality of life. In this role, you will be an essential member of the clinical team, providing expert care to a diverse patient population. You will use your advanced clinical skills to assess, diagnose, and treat patients autonomously while working collaboratively with a team of healthcare professionals.
Key Responsibilities:
Clinical Assessments: Conduct thorough patient assessments and diagnose a wide range of acute and chronic conditions.
Treatment Planning: Develop and implement evidence-based care plans tailored to individual patient needs.
Prescribing: Independently prescribe medications and treatments (V300 qualification required).
Record Keeping: Accurately document patient care using the Adastra system.

Requirements:
NMC Registered Nurse or HCPC Registered Professional with an Advanced Clinical Practitioner qualification.
Independent Prescribing Qualification (V300).
Proficiency in using Adastra for patient record management.
Experience in primary care, urgent care, or community healthcare settings.
