​Are you a skilled Advanced Clinical Practitioner (ACP) with a passion for patient care and experience using Adastra? In this role, you will be an essential member of the clinical team, providing expert care to a diverse patient population. You will use your advanced clinical skills to assess, diagnose, and treat patients autonomously while working collaboratively with a team of healthcare professionals.

Key Responsibilities:

• Clinical Assessments: Conduct thorough patient assessments and diagnose a wide range of acute and chronic conditions.

• Treatment Planning: Develop and implement evidence-based care plans tailored to individual patient needs.

• Prescribing: Independently prescribe medications and treatments (V300 qualification required).

• Record Keeping: Accurately document patient care using the Adastra system.

Requirements:

• NMC Registered Nurse or HCPC Registered Professional with an Advanced Clinical Practitioner qualification.

• Independent Prescribing Qualification (V300).

• Proficiency in using Adastra for patient record management.

• Experience in primary care, urgent care, or community healthcare settings.
