Job description
We have partnered with a cutting-edge healthcare partnership and training practice in South-East London. They have a list size of approx. 21,000 patients and are dedicated to improving patient services and training and developing clinicians for the future. They are actively seeking a new Clinical Pharmacist to join their diverse and experienced team of healthcare professionals.
Position Details:
Job Title: Clinical Pharmacist
Job Type: Full time/part-time hours available from Monday – Friday
Location: South-East London
Salary: £45,000 - £55,000 per annum, depending on experience, with an extensive benefits package including learning and development opportunities.
Additional Services:
Complete range of medical services, including maternity care, baby clinics, immunisations, general medical and surgical care, minor surgery, contraception, screening, and health promotion for both men and women.
Support Team:
Work alongside four GP Partners, Salaried GPs, Nurse Practitioners, Practice Nurses, Physician Associates, and Healthcare Assistants.
About You:
GPhC registered pharmacist with experience working within a GP Practice setting.
Experience and an awareness of common acute and chronic conditions.
Effective communication of complex information via various channels.
Strong interpersonal, organisation, and IT skills, capable of challenging managerial and clinical practices.
May hold or be working towards an independent prescribing qualification is desirable but not essential as full training provided.
Main Duties:
Work as part of a multi-disciplinary team to clinically assess and treat patients, utilising expert knowledge of medicines for specific disease areas.
Manage care for patients with chronic diseases, conducting clinical medication reviews to proactively handle complex polypharmacy cases.
Ensure accurate recording on clinical systems to meet QoF compliance, while contributing to meeting targets and enhancing services.
Provide leadership on medicines optimization, quality improvement, and adherence to guidelines, while maintaining a clean and effective working area.
