in the United Kingdom
An up to date CV
detailing a minimum of the last 10 years working history with no gaps or gaps justified in written form (please supply in MS Word format)
2 x Referee details
from the last 2 or most recent positions on your CV
DBS process started
(if you have registered with the DBS Online Update Service then please supply the original DBS certificate used to register) or DBS dated within 12 months
Evidence of your current Indemnity Insurance cover
i.e. proof of membership letter issued by your insurer. This must include a description of the work you are covered for i.e. number of private sessions and/or out of hours work
Evidence of your current Professional Registration
with the NMC
